- Do you take pain relievers preemptively during your period to prevent or help to lessen pain?
- Has pain during your period become progressively worse?
- Does the pain you experience during your period cause you to alter your schedule or change your daily activities?
- Does pain persist even between your periods?
2. DO YOU EXPERIENCE PAIN DURING SEX?
- Does having sex with your partner make you anxious or fearful due to the pain?
3. DO YOU HAVE A FAMILY HISTORY OF ENDOMETRIOSIS?
- Does your mother, sister or aunt have endometriosis?
4. HAVE YOU HAD TROUBLE GETTING PREGNANT?
- Are you under the age of 35 and been trying for more than 12 months with no success?
- Are you over the age of 35 and been trying for more than 6 months with no success?
5. DO YOU HAVE OTHER SYMPTOMS OR HEALTH CONCERNS THAT COULD BE RELATED TO ENDOMETRIOSIS?
- Do you experience diarrhea, constipation, painful urination or blood in your urine?
- Do you experience bloating or nausea?
- Do you have abnormal periods that are more than seven days or come with a heavy flow?
- Have you experienced fatigue or a change in your personality?
